Breast augmentation with implants is one of the most frequently requested procedures in aesthetic surgery today. The reasons for this are as varied as the individual needs of the patients. For women whose breasts are only slightly developed or have changed due to significant weight loss, pregnancy or breastfeeding, for example, breast augmentation with implants (augmentation plastic surgery) can improve their body image and self-confidence.

In this procedure, silicone gel implants are usually placed through a precise incision in the underbust crease. The implant is selected individually, depending on the patient’s wishes and anatomical conditions. Modern, high-quality silicone implants are very comfortable to wear. They often promise a lifelong guarantee, but they have a limited service life and may therefore need to be replaced.

We also offer lightweight implants made of microspheres, which are particularly suitable for weak connective tissue, but also for sporting activity and larger volume requirements. We also use breast implants that dynamically follow the woman’s natural movements thanks to their ergonomic silicone gel. In special cases, breast implants made of polyurethane can also be used.
 
In addition to the classic method with implants, in our practice clinic for plastic and aesthetic surgery we often carry out breast enlargement with your own fat, which is often requested as a more natural alternative, but is not suitable for all patients. In comparison, breast implants enable defined breast enlargement in just one operation.
 
Breast enlargements are also offered as part of gender equalization operations for male to female transidentities. Here it is important to take the special anatomical conditions of the patient into account when planning the operation.

Procedure for breast augmentation with implants

Before a breast augmentation with implants, our plastic and aesthetic surgery specialists, Dr Andrea Wolff and Dr Isabel Edusei, will carry out an individual consultation. The patient’s needs and wishes are discussed together and a thorough examination and consultation are carried out in order to clarify the anatomical preconditions and achieve the best possible and most natural result. The choice of the right breast implant is of great importance here. We will introduce you to various implants. By using trial implants, a preliminary result can even be simulated before the final breast implant is inserted.
 
Our plastic and aesthetic surgery practice is manufacturer-independent and only uses breast implants from renowned manufacturers that are known for their first-class quality standards. These implants consist of a cohesive silicone gel that imitates the natural breast tissue as closely as possible.
The specially optimised shell surface of the breast implants used ensures better tissue fixation, which reduces the risk of implant rotation. In addition, the surface enables a small access point when inserting the breast implants. This significantly reduces the risk of complications such as capsular contractures, double capsule formation or late seroma in the implant site.
 
Some of the breast implants we use offer special innovations, such as significant weight reduction (so-called B-Lite implants or lightweight implants) without compromising on resistance. These are particularly suitable for cases in which the skin-soft tissue mantle on the breast is already sagging and therefore cannot bear much weight.

There are various access routes to choose from for breast augmentation with implants, although not every incision is suitable for every patient. The most commonly used method is the incision in the lower breast crease (inframammary or submammary). This allows a clear overview of the surgical area, especially in revision procedures, and ensures gentle placement of the implant while taking the mammary gland tissue into account. This not only favours the healing process, but also preserves the breast’s ability to breastfeed. In addition, the incision can be placed in such a way that the scar is barely visible, which is particularly advantageous for women with a small areola.
 
Alternatively, the incision can be made in the area of ​​the nipple (periareolar) for breast enlargement with breast implants. This is particularly recommended for breast deformities or an unclear under-breast fold. Here, the incision is made in a semicircle and ideally exactly at the transition between the areola and the breast skin. This approach also allows the nipple to be tightened and reduced at the same time, if necessary. In the case of a planned breast lift, the implant can ideally be inserted in the area of ​​the tightening incision in order to achieve a harmonious overall result. The option of an armpit incision (axillary) is not used in the Wolff & Edusei practice clinic for breast enlargement with implants for various reasons and is therefore not discussed further here.
 
Breast augmentation with implants is performed by our plastic and aesthetic surgery specialists on an outpatient basis in our practice clinic. The operation usually takes between 1 and 2 hours and is performed under general anaesthetic.

After breast augmentation with implants

After breast augmentation with implants, a recovery period is of great importance, during which all movements should be carried out carefully and without strain so that the implants can heal in their implant site without any problems. The arms should not be moved above shoulder height and it is necessary to maintain a supine position while sleeping for a period of at least 6 weeks.
 
Early mobilisation on the day of breast surgery is important. You should avoid adopting an overly protective posture, which can lead to painful muscular tension. In the first few days after the operation, daily showering is only possible from abdominal level, while areas above the abdomen can be gently cleaned with a ‘cat wash’. A compression bra and, if necessary, a Stuttgart belt must usually be worn for 6 weeks after breast augmentation to support the healing process.
 
Around three months after the procedure, the treated area should generally be sufficiently healed to be able to bear full weight again. Nevertheless, these are only average values, and an individual consultation with our plastic and aesthetic surgery specialists, Dr Andrea Wolff and Dr Isabel Edusei, is of crucial importance.
 
Although no severe pain is generally to be expected after breast augmentation with implants, the sensation of pain is of course always perceived individually. We therefore provide painkillers in accordance with a treatment plan specially created for you.
 
The first follow-up check takes place on the first day after the breast operation, when the dressing is also changed for the first time. The insertion of so-called Redon drains to drain wound fluid during the operation is rarely indicated. Further follow-up appointments, including further dressing changes, will then follow at regular intervals to carefully monitor your healing process.
