Gynaecomastia (female breast)
The term gynaecomastia denotes an excessive manifestation of the male breast. This is often extremely unpleasant for the man affected. In most cases his well-being is impaired, especially if his chest assumes a shape that is more or less female. The situation is often exacerbated by physical complaints such as feelings of tightness and oversensitivity of the nipples. As a matter of basic principle, the options for treatment in cases where a man's breasts grow like this – for example a gynaecomastia operation involving liposuction, possibly in combination with the surgical removal of the gland – are good.
This undesired »man's bosom« is usually caused by one of two things:
- Often, the enlargement of the breasts has come about because of an excessive accumulation of fatty tissue. In most cases, the root cause is excess weight, in combination with an altered hormonal balance.
- Only very few of those affected actually have this over-developed chest because of »true gynaecomastia«. In these cases, the body of the mammary gland is unnaturally enlarged, either for genetic or hormonal reasons, or on account of long-term medication.
Your clinical picture is determined by various different examination procedures (physical examination, laboratory diagnostics, ultrasound examination). If there is evidence of a "true gynecomastia", a reimbursement can be requested from the responsible health insurance company.
Gender reassignment woman to man (mastectomy)
Regardless of their sexual orientation, many people feel as if they are imprisoned in the wrong body.
Normally, the acknowledgement of »woman to man« transsexuality requires a psychological report with surgical indication, an official change of name, and hormonal therapy with proof of having lived for a prescribed period in the other gender, so that an application can be made to your health insurance provider requesting coverage of the costs of reassignment operations.
The series of gender reversal operations is usually begun by removal of the female breast (mastectomy), the latter being the main visible stigma for the patient. Which surgical method can be used in your particular case (e.g. perimamillary access or free nipple transplantation) depends on your baseline findings.
During the procedure, you can choose from some options, for example a microsurgical reconstruction of the penis, a so-called »penoid«, or a removal of the uterus and ovaries.

The so-called funnel chest is a common clinical picture in men. From puberty onwards, this can often lead to permanent feelings of shame vis à vis the female sex, but also vis à vis other men.
In these cases, the sternum (breastbone) is sunken or retracted to a greater or lesser extent. If the emphasis is on functional problems, it becomes necessary to widen the ribcage by surgical means. However, the focus is mostly on cosmetic impairment.
Training the chest muscles may help, but in the great majority of cases it is unfortunately not sufficient to hide the asymmetry. For this, there are various different microsurgical treatment methods which leave few scars, such as the fitting of tailor-made implants and / or autologous reconstruction with fat from your own body.
